I like Co-Pilot Live, North America. I have it on my iPhone, and it does provide live, turn by turn directions with POI's and all that good stuff. I also have it installed on my iPad in the 2X mode, rather than re-buying the "HD" version, and it seems to work just fine.
The only issue I could see is that the iPad really isn't a good size for mounting somewhere in a car, which means you might leave it on your seat or something like that, and it could potentially be a big distraction trying to look towards your seat.
Anyway, Co-Pilot is the only one I've used, mostly because of the price. For a hundred bucks, I'd rather have a stand alone GPS than an app any time. That's just me though - Co-Pilot runs about $40 for the iPad version I believe.
There are also a few free versions out there now. I don't think that they do the turn by turn with voice, but you might want to check them out for a comparison, so you know what you definitely do want, and what may turn out to be less important for you.
I was just looking at the GPS apps yesterday, but haven't bought one yet. The one that caught my eye was "MotionX GPS Drive HD," which has turn by turn voice voice directions. The app is $2.99, and requires a fee for the voice directions-- $2.99/month or $24.99/year. You can buy it on a monthly basis if you want--no contract required. (Much like the AT&T unlimited data plan was supposed to be...but don't even get me started on that!)
Anyway, it has pretty good reviews in the app store, and you can't beat the price.

No, I haven't bought one yet. Still just browsing. I have so much fun looking at all the apps in the AppStore that I don't have time to actually buy and use many.

They have turn-by-turn voice driven directions and you have a choice of 3 voices to use. I used it at the same time as my regular stand alone GPS driving through rural TN & NC, and metropolitan GA, and it never lost the signal, and while it occasionally took a different route from the standalone GPS, it always got us to where we were going. (We would swap which one we got the directions from on different trips out, to see the different routes) I'd have to say that I highly recommend Co-Pilot. I also believe that it is the only GPS app that is fully iOS4 compatible at the moment. I'm sure the others will catch up, so if you have a favorite or already purchased app, I wouldn't worry, but if you are looking for one on your new iPhone 4, Co-Pilot is great.
